Primary Source Verification Search

Northwestern Medicine has provided this primary source verification search for other hospitals, healthcare organizations and credentialing agents to expedite a response to your request. This standard verification letter can be used for all practitioners, and will be the only response issued for those that have been inactive for over 24 months, have low clinical activity, and are a telemedicine or APN/CRNA practitioner.
